The Palmetto Palace Mobile Health Unit will be at Revive Charleston (1527 Remount Rd North Charleston SC 29406) from 2 - 5pm on Sunday, April 28th offering the following services for Medicaid and/or underinsured/unfunded patients:

• Chronic Care Management (Diabetes, Hypertension, Asthma, Hypothyroidism, Anemia)

• Women's Health (Pap Smears, Referrals for Mammograms, Sexually Transmitted Disease (STDs))

• Mental Health Navigation (Patient Health Questionnaire-9 (PHQ-9))

• Free on-site medications plus for every lab sign-up free mixed bag of groceries! Open to individuals 15+.

Please bring an identification card if possible as well as any medications you are currently taking.
