We are pleased to offer a new location for the Palmetto Palace Mobile Health Unit on the 4th Sunday of each month! Starting Sunday, August 27th, 2023, we will be at Revive Charleston (1527 Remount Rd North Charleston SC 29406) from 1 - 5pm offering the following services for Medicaid and/or underinsured/unfunded patients:

• Chronic Care Management (Diabetes, Hypertension, Asthma, Hypothyroidism, Anemia)

• Women's Health (Pap Smears, Referrals for Mammograms, Sexually Transmitted Disease (STDs))

• Mental Health Navigation (Patient Health Questionnaire-9 (PHQ-9))

Free on-site medications plus for every lab sign-up free mixed bag of groceries!

Open to individuals 15+.

Please bring an identification card if possible as well as any medications you are currently taking.
